Program Participation Consent*

By checking this box, I am electronically signing my application and fully agree with and understand the below consent statement.

I understand that an electronic signature has the same legal effect and can be enforced in the same way as a written signature. My consent for my child and or myself to participate in this program is completely voluntary. I understand that by signing this agreement, I agree to save and hold harmless and indemnify The Yunion Inc to the extent permitted by the law from all liability, loss, cost claim or damage whatsoever which may be imposed upon or incurred by said parties because of the participation of the minor in the program and release said parties in this regard on behalf of both the minor and the parents or legal guardians as well. I also grant permission for the use of images and the voice of the student and or parent while the student and or parent is involved in program activities, for promotion of The Yunion Inc., whether produced by The Yunion Inc. or commercial media outlets. I agree that my child or myself may take part in data collection methods (surveys, questionnaires, etc.) that will assist the program in determining if my child or myself was satisfied with services and if set expectations and outcomes of the program were met. Further, I understand that Yunion programming is provided at low or no cost through grantor agencies such as the Michigan Department of Health and Human Services, Wayne County Health, Human and Veteran Services, the Michigan Council for Arts and Cultural Affairs, and other private and foundation funding. My child's participation in the program may be funded through one or several of these funding sources. Parent Orientation & Training Day Policy*

I agree to the Parent Training Day policy.

In order for students to receive a scholarship, it is mandatory for all parents to attend the Parent Training & Orientation. You will not only receive vital information about camp, but will be educated on the current trends in mass media that affects youth today through our "Parental Advisory" Training. Upon completion of the training, your child will receive the scholarship award letter. IMPORTANT: Please note: failure to attend will result in the revoking of the scholarship, and the balance of the original camp price of $597 will be owed. PLEASE NOTE: This year's DATE and TIME for the Parent Open House & Training Day is publicly available on the Keys 2 Life Summer Camp Website at www.keys2lifesummercamp.com. In addition, at some point after you register, you will receive a reminder email containing the details regarding this mandatory parent Open House & Training Day.

Refund & Cancellation Policy*

Refunds will be processed up to June 17th. Due to the significantly reduced cost of the camp, please note that once camp begins, we cannot process refunds or deductions if students are absent for any reason. There will be no exceptions to this policy.

Keys Camp COVID19 Safety Practices for Face to Face Programming*

I have read the Keys Camp COVID19 Safety Practices for Face to Face Programming

All staff and participants will be masked at all times.
The Yunion shall require staff to receive a negative COVID-19 test within 14 days of returning to work.
The Yunion shall provide staff with training that promotes behaviors to reduce the spread of the virus such as, but not limited to, staying home when sick, respiratory etiquette, and practicing hand hygiene (including hand washing with soap and water regularly).
Yunion staff shall participate in daily temperature checks by utilizing temperature kiosks placed in various locations.
Yunion staff shall be required to affirm they are not experiencing Covid-19 symptoms prior to entering a building or office space by completing health screenings.
The Yunion shall provide all necessary Personal Protective Equipment (PPE) for staff persons when in any face to face program location.
Yunion staff shall comply with posted signage promoting social distancing and safe practices.
The Yunion shall limit the sharing of equipment, supplies, and other items between staff and students.
The Yunion shall limit non-essential staff from visiting any face to face program location.
All inside rooms will be sanitized, implementing the sanitizing procedures established by the MDHHS and CDC. Established COVID safety protocols will be maintained.
Tables, desks and other surfaces will be cleaned and disinfected frequently. Students will not share items, each student will have their own supplies, if applicable
Handwashing or hand sanitizing stations will be provided throughout the face-to-face program event to encourage healthy hand hygiene.

Release of Academic Records Consent*

I understand that one of The Yunion's primary goals with their programs, such as Keys 2 Life Performing Arts Summer Day Camp, is to increase each student’s academic performance. Academic records are used for program evaluation purposes only. The Yunion has an obligation to keep your personal information, identifying information, and records confidential.
