Aaron behavior at home has been really good. He is complaining less and keeping his room clean. He cleaned up without being told and maintaining his room

His behavior has been pretty good. For a while, Aaron seemed down and distant, but I’ve seen something in him shift. I’m seeing him smile and his relationship with his father gets better

He has two D’s one on science and math. He do struggle in these two classes but he said he will pull the grades up by final grade marking

He is getting much better at responding but we would like him to take more inktive with his chores.

He struggle when it comes to correction. For example if we sag that the garbage is overflowing and trash fall on the floor and this continue to be a problem, he hold his head down or think that we have a negative opinion of him overall. He doesn't like any criticism. This is a work in progress.

Mason hasn’t been Accountable for his actions. He blames other people for his consequences and always tries to get out trouble by deferring and deflecting to other people.

Mason was suspended the last three days of schools for fighting. Tell weeks before he was suspended for fighting, for 2 days. Mason has 2 D+ on end of year report card. Blames his teachers and kids in his class for not preparing or completing his make up work.

Mason, is still having some behaviors when he doesn’t understand a situation or he perceives things at being over looked or he is being mistreated. He is so much better…last week was first time we talked through what choices he wants to make and he said he doesn’t want emotions to rule over him. I’m so happy for him.

Mason works very hard in school both academically and behaviorally. He tries to put forth full effort, and K appreciate that.

Behavior was ok this week until tonight when he was practicing his Cave materials, his sister came downstairs, Blake got angry, grabbed his Cave binder and bag, went to his room and threw the bag, all the papers in the binder and the binder against the wall in anger. All because his sister came downstairs during his practice.

Blake’s teacher complained that Blake has not been doing his work and is behind in her class. Also, Blake had to have a conversation with the principal about him and a little girl’s inappropriate behavior at school.

DJ's behavior remains disrespectful. He says that he has a sad face when he is with me because I am always fussing. I attempted to explain to DJ that my fussing is because of his unacceptable behavior.

There hasn't been any school. The failing grades received on the final report card were discussed with the Assistant Principle Mr. Parks and he explained that because DJ has an IEP he should not have received a failing grade. The teachers were contacted and they also agreed and the changes are being made per IEP laws. This does not mean DJ is not failing. It simply means they cannot fail him.

He use to but not so much any more. However, I have told him and reiterated today that I will be forced to use physical "touch". I am very tired of his disrespect toward myself and my wife who has only been a positive reinforcement in his life.

DJ has not been following directions when he's with me. I have to ask him more than 1 time to complete a task and when asked why he always has an unacceptable excuse. He has disrespectful to my wife as well. He's been advised by his mother and her family that he does not have to listen to her and she is not his mother. This has caused serious conflict in the home.

DJ has not attended summer school. However, I did receive his report card for the end of the year and it was not good. I will send a copy to Justin this morning.

DJ has become a child that I/nor my side of the family recognize. He has a nonchalant attitude. He is non apologetic for his actions and/or behavior. On several occasions I've had to ask to complete task more than 2 times. I finally told him a few days ago there would be repercussions that I was certain he would not like.

DJ is quiet/not talkative on most days. He converses when spoken to or asked to engage. If we allow him, he will stay in his room all day unless he is eating or going out. For the most part he does what he's asked the first time but on some occasions he has to be asked to perform a task more than once. DJ likes to be a jokester or the funny person. That is how he has become what he considers the cool kid in school and that has also gotten him into trouble several times. DJ doesn't do well with the word NO. He pouts and has to be asked what is wrong often. We have decided to ignore the behavior and move forward. He has been advised that he will not always have his way.

Dwayne's behavior in school has been questionable. Before the end of the school year he received a suspension for play fighting in the restroom with a peer. DJ has been reprimanded by having his cell phone taken and hair cut. He admits that his attention span is not long and he gets distracted easily. We have asked his teachers to sit him close to them and given them permission to contact us as needed.

Not as much as we'd like. When we discussed his progress report with him that was not to our liking he cried but did not want to share why he was doing so. We asked several questions and denied all that were asked such as, are you disappointed in himself?, why haven't you learned from earlier behavior that got you into trouble? Why aren't you taking school more seriously?, will you be embarrassed if your peers have moved on to the 9th grade and we must chose to leave you in the 8th grade to assure you are learning? how will you make this better? No answers to any of these questions, yet the tears continued to flow

He generally minds well. However, he would prefer to be alone(in his bedroom) if he isn't doing things with friends. Even when family is visiting he has to be told to engage. We are still working on getting him to do chores without being asked but because he knows its the right thing to do. I am trying to unlearn some of my behaviors such as yelling at him to get him to do things. It's not the way I want to communicate with my son.

DJ's behavior has never been a concern until he was bullied in 7th grade and transferring to a different school for 8th grade. DJ is quiet for the most part until he becomes comfortable with those around him. since he's been in the new school he's tried to make a name for himself. He's done so and it hasn't been good. He's gotten in trouble several times for talking back to a teacher, going against the grain of what he knows is right, and behavior that can be seen as bullying. DJ has been excluded for horse playing in the restroom and not doing the work that was given. In spite of that, his teachers see the good in him and have spoken with him and encouraged him to do better. We have continued communication and transparency about his behavior and the consequences of that behavior, i.e. hair cut off and participation in 8th grade activities taken away, and cell phone disconnection. DJ has an IEP but he has the resources and support needed to do better. He has scheduled tutoring on Tuesdays as well. I am unsure what else to do. I admit that I get discouraged. I remain prayerful.

When my son is with me I am always asking him why he goes directly upstairs to his room instead of staying downstairs to connect with me and his Bonus Mom. Unless we are horseplaying there is little communication unless initiated by me or someone else in the home. DJ is reserved and always appears that he doesn't want to be present(in our home). I rarely experience negative behavior from my son while in my care.

My son's behavior and academic performance in school lately has been unacceptable. DJ seems to care more about being the class clown and being cool than doing what is expected of him by his teachers or my wife and I. I was called 2 times in 2 weeks about him not being prepared for class, acting silly and missing assignments. DJ has a 504 plan and tutoring that he misses with the permission of his mother. He has a C/C- average at best so he needs all the assistance he can get!

He has been quite anxious and hearted himself for not fighting back after the boy attacked him. He has been a very poor sport and crying easily with minor injuries during practice. A bigger problem is that on Saturday Jeremiah asked me to log in to my parent account for his Nintendo Switch, then, without my permission or knowledge, he charged $100 to my credit card and bought new games.
I took away his Nintendo for a month, with the possibility of him earning it back before then.
I’m also not pleased that he asked me who Bonnie Boo is. I had no idea. Turns out it’s actually Bonnie Blue, a porn star he learned about AT THE CAVE. I’m pretty upset about this.

In general I would say it's been good. However, it seems that Jeremiah and Maggie (his mom) seem to have an angry blowup a day or two after I submit this form. Then it's better again for ten days, then I fill out the form again and they have another argument. But overall, I would say pretty good. Maggie would probably disagree with me.

When I put my foot in his but he does what he is supposed to do. So I guess we are working on that muscle.

Liam is 12 we have good days and not so good days. For the most part he is ok. Yes I have to say things for him to do twice and I yes I have to ttell him to do things he is not at the point of doing it when it needs to be done...i.e., trash cleaning his room

Well he was behind in his work so he had F's in 3 classes; As of today, he has completed the work he missed and his grades are no longer F's, as of last week he has a C in one class a B in the other and a D in the third class. I know he is capable of consistency when it comes to his school work. I recognize I must do better as well. Because when I got on his a$$ he got that work done and those grades up...so there is that

It’s okay, when his older brother comes over who doesn’t live with us it seems like he follows him and becomes extremely silly. This is the same thing he would do in school with other kids. We are trying to help him to not be so easily influenced by other kids when they are around, but it is hard.

As mentioned, when in school the constant feedback was that he would joke and speak in appropriately with young women. Me and his dad have talked to him so much. Can the cave please assist with this? This is honestly my biggest concern for him going back to school.

Omari continues to demonstrate significant behavioral concerns at home. The primary concern is an ongoing pattern of stealing despite repeated discussions, consequences, and interventions. He has taken cash from me, personal property from his brothers, and used his brothers' debit cards without permission to purchase snacks. He has also stolen food items from local stores.
A particularly concerning incident occurred when he stole and consumed a Quest protein bar despite having known food allergies. He experienced a severe allergic reaction that required emergency medical treatment and hospitalization for the evening. Even after this incident, there has been little evidence that he understands the seriousness of his choices or the potential consequences of his actions.
Additional concerns include:
Entering personal spaces, including purses, bedrooms, and belongings, without permission.
Refusing to leave restricted areas when redirected.
Frequent arguing, defiance, and resistance to authority.
Intentionally provoking and antagonizing siblings and family members.
Persistent boundary-testing behaviors despite clear expectations and consequences.

Students were engaged in horseplay while setting up a football play; however, Omari was not actively participating in the activity. He ran up behind a student who was already wearing a cast on his wrist and forcefully pushed him to the ground. His actions were disruptive, unsafe, and resulted in harm to an already injured student.
Following the incident, Omari was removed from class by the principal. During the administrative intervention, he became increasingly combative, loud, argumentative, and disrespectful in both his language and tone. His behavior escalated to the point that I was contacted at work and required to leave my job site to come to the school and take him home.
Academically, Omari is capable of performing at a higher level than his current work reflects. However, he is not consistently completing assignments or submitting work on time. He continues to accumulate late and missing assignments, resulting in frequent emails and reminders from teachers regarding incomplete coursework.

Behavior at home has declined significantly. There has been an increase in argumentative and defiant behavior, as well as poor decision-making. He has left the home without permission on multiple occasions, including going to a nearby location without notifying an adult. There has also been an incident involving unauthorized use of a sibling’s debit card to purchase items. Additional concerns include failure to complete assigned chores, dishonesty, use of inappropriate language in front of adults, and disruptive behavior during family routines (e.g., consuming a disproportionate amount of shared food without regard for others). These behaviors indicate decreased impulse control, poor judgment, and a lack of accountability.

He is currently returning from a suspension related to a physical altercation. There are ongoing concerns regarding peer interactions, including poor decision-making in friendships and noncompliant/disruptive behavior (particulary in unstructured settings- lunch/recess; hallway passing, etc). He has been described by staff as noncompliant at times. While there has been some improvement in academic performance, follow-through on assignments remains inconsistent. He also left school early recently citing a headache, which may be related to avoidance rather than a medical concern.

However, over the past few weeks, his follow-through on household responsibilities has declined noticeably. He has required increased prompting and direct supervision to complete basic chores. In many cases, he needs to be physically redirected from his current activity to begin a task and remain on task. When left to work independently, tasks often remain incomplete, as seen in ongoing concerns such as an unmaintained bedroom and laundry not being managed.
Our goal is to support Omari in building greater independence and accountability, specifically working toward completing tasks and meeting expectations with minimal prompting, without delays or avoidance behaviors.
However, over the past few weeks, his follow-through on household responsibilities has declined noticeably. He has required increased prompting and direct supervision to complete basic chores. He has picked up this new habit of sulking anytime he does get his way, also when I try to make general conversation, or anytime he's in any other state than perfectly happy. It comes along with me having to repeat myself for him to respond to questions and this past weekend it resulted in a pop in the mouth because of how bad his attitude was. He has since straightened up but I wish I hadn't have gone that route.
