Struggling (1/5)
01. Behavior at home
He is still continuing to eat in his bedroom and leave dirty dishes there. I have to remind him multiple times to not do that and to take the dishes downstairs. He has gotten better about doing what I ask. When I ask, however, the initiative to do the things that need to be done when he sees them has not been happening. I would like him to improve on that. For instance, when the garbage is overflowing, he should just take it out and not wait for me to ask.
02. Behavior & academics at school
I received an email from his gym teacher. It did come on a day when he forgot to take his medication. Please see below. "Dear Ms. James, Carsen is back in PE for this quarter. The first two days went well, but today did not go well for him. He was unsafe in class and hanging from the bar that supports one of the side backboards in the upper gym, and later grabbed and held onto a basketball net. Students have been told repeatedly that this is not okay behavior.
Carsen also had his hands on other students in class, used inappropriate language, and was talking over me when I was talking to the class at the start of class for directions. When he was asked to sit out temporarily, he wanted to argue about it and then kept asking me questions and singing at me. He took a basketball out of the storage room without permission, and opened a stairway door when he was supposed to be sitting out. When he was allowed to play in the game, he was not playing by the rules, and prevented others from playing the game. I reminded Carsen that he needs to follow school and classroom expectations. I told him that is not part of being safe, responsible, and respectful in class, and he will sit out of activities and lose class credit. He replied that it didn't matter because he would still pass the 8th grade. He did receive a VOE for his choices today. I'm not sure if Carsen wants to be in this class this quarter, or what other options would be available for him this quarter, but I told him he could speak with his counselor to find that out. I appreciate your time and support, and your support of Carsen. Please let me know if you have suggestions of ways to help support him here at school.

Struggling (0/5)
01. Behavior at home
Carsen's has his days. He has been very into his phone socializing and on the video game. I have had to remind him multiple times every few days to stop eating and drinking in his room- there have been collections of dishes and food left for days. He has been doing better about making his bed and keeping up with laundry but improvment is needed. He needs to be asked multiple time to do something because he is distracted by other things. When called, he has been saying "what" and needs reminders to respond with "Yes?"
02. Behavior & academics at school
he has had a few calls and email from the school and Athletic director about his behavior- he has 2 classes with D's that he had multiple missing assignments that he says have now all been turned in.

My most recent interactions with my Bonus Son were AWESOME. We talked, laughed and interacted much like we did when he was younger.
02. Behavior & academics at school
DJ has improved. He has a 504 plan so he receives school assistance and tutoring from an honor student that attends Cranbrook/Country Day on Tuesdays through an organization name, Cradle to College that takes place at the Inkster Recreation Center.

Struggling (1/5)
01. Behavior at home
Hello, Dwayne is with us every other weekend and during the week when we drop off and pick up from the CAVE. The rules are different in our home then at his mothers home. Dwayne is generally allowed to make decisions at his mothers home and NOT in our home. We don't ask for preferences and that usually causes an issue. Dwayne either has a pout on his face and/or chooses to be reserved with little conversation. He still has to be told to take the trash out when it's full and wash his used dishes when he is done.
02. Behavior & academics at school
Dwayne received a call home 2 weeks in a row from different educators. The first call was regarding being ill prepared for class and the second call was for missing assignments, talking back to the educator and challenging her. My husband picked him up from school early on this day to reprimand him. Dwayne was given a whooping with a belt for misbehaving.
03. Responds first time
We usually do not have to ask Dwayne more than once to complete a task. It is not often that a second request is made.
04. Sir/Ma'am
No
05. Emotional improvements
Dwayne usually has a scowl on his face and is not smiling unless he's doing something he likes, initiates or speaking with friends on the phone and playing the game. Dwayne is not a child that gives us a challenge with back talk or misbehavior in the home.

01. Behavior at home
DJ has been with us (his Dad & I) during the break. The week started off ok. I picked him up on Friday around 5pm. He headed upstairs per usual to his bedroom. He's usually quiet and comes downstairs if called, to eat, shower or leave. DJ is very reserved when he visits with us. This week I found out from DJ that he cannot refer to me as Mom Val because his mother over heard him calling me this, became upset and advised him not to call me that again. My husband and I assumed this was happening and DJ confirmed our suspensions. DJ apologized when I asked him why he called me Val. After he told me what his mother advised I hugged him, told him that he wasn't in trouble and that he should respect his mother's wishes when he's around her. He seemed relieved.
The remaining part of the week has been uneventful.

Struggling (2/5)
01. Behavior at home
Difficult. More times than not, Harper’s choices involve disobedience, disrespect, and doing the opposite of what is asked.
02. Behavior & academics at school
It is the same at school. His grades have fallen. He is failing one class and has c’s in almost all of the rest. He lies about having homework, he doesn’t turn it in when he does it. I get a call home from school regarding bad choices 3-5 days a week. Most recently he intentionally stuck his foot out to trip the students running through a congratulatory student line.

Over the past week, Hudson’s behavior has been better than previous however, he had a violent attack at school. Experiencing a larger amount of anxiety.

Needs Attention (3/5)
01. Behavior at home
Generally good. But we’ve had some instances of talking back.
A bigger problem is that on Saturday Jeremiah asked me to log in to my parent account for his Nintendo Switch, then, without my permission or knowledge, he charged $100 to my credit card and bought new games.
I took away his Nintendo for a month, with the possibility of him earning it back before then.
I’m also not pleased that he asked me who Bonnie Boo is. I had no idea. Turns out it’s actually Bonnie Blue, a porn star he learned about AT THE CAVE. I’m pretty upset about this.

01. Behavior at home
In general I would say it's been good. However, it seems that Jeremiah and Maggie (his mom) seem to have an angry blowup a day or two after I submit this form. Then it's better again for ten days, then I fill out the form again and they have another argument. But overall, I would say pretty good. Maggie would probably disagree with me.
02. Behavior & academics at school
Generally good, but he has a D average in science. He seems to try just hard enough to get through, but never enough to actually remember anything after the test or assignment.
03. Responds first time
We frequently have to ask him to do things like chores or brushing his teeth, etc.
I think it's because he doesn't like to be interrupted.

Struggling (1/5)
01. Behavior at home
Generally, it's been pretty good. He is still reluctant to do his chores and still argues about taking a shower.
02. Behavior & academics at school
Behavior has been great. His teacher named him "Person of the Day" one day last week. This is an honor that not earned every day. The teacher says she picks one student at the beginning of the day and watches him/her all day. Some days the student earns the award, other days not. But on this day, she watched Jeremiah and his behavior impressed her. Not sure if I mentioned this in my last check-in, but his teacher also told me something else good about Jeremiah. She said he doesn't know it, but he has helped bring his classmates together this year. She said that last year, when the cohort was in 4th grade, the group was divided into cliques that didn't associate with each other. But with Jeremiah in the class, he brings joy and humor and includes everyone. Now they all play and get along. What a blessing to hear that!
03. Responds first time
He is usually reluctant to do what we ask him to do. We frequently have to ask him twice.

01. Behavior at home
Pretty good. Some reluctance to follow rules, some talking back. He misses his mentor from Easter Seals, who abruptly quit, breaking his commitment to mentor Jeremiah.
He complains a lot, including on Saturday, when we went for a walk on a nature trail. He hates going for walks or bike rides and complains when we do it.
He complains a lot, including on Saturday, when we went for a walk on a nature trail. He hates going for walks or bike rides and complains when we do it.
02. Behavior & academics at school
Jeremiah's teacher told me the most wonderful thing on Friday. She said that Jeremiah doesn't realize it, but he has brought his whole class closer together just by his personality and by being himself. She said that last year (before Jeremiah joined the school), the fourth grade class was divided into cliques that didn't get along with other kids. Now that Jeremiah is here, they all get along better. He includes everyone. I was crying to hear that. That's my boy! But he doesn't realize he is loved. That's sad.
03. Responds first time
He often responds with a tone of voice that sounds irritated or defensive or angry, as if we are asking him to do something hard, or to spend his whole day doing chores. We often have to ask twice or more (or to remind him), even for simple things like deodorant, taking a shower, or taking his dirty laundry to laundry room.

He got detention Friday for being disruptive in class (talking and joking while the teacher was talking). Another boy in his 5th grade class got suspended for making sexual comments. The same boy has been talking about porn to Jeremiah and the other boys in his class since October. I’m happy that Jeremiah was not involved, but I would like it if he would be assertive and tell other kids to stop when they are not doing what is right.

Struggling (2/5)
01. Behavior at home
Not the best. He's often disrespectful. Responds to me or his grandma with yea whatever or ok bro. He often yells when he's frustrated and says shut up. Argues with his sisters (older and younger sister) and bucks at them (pretending he's going to hit her) he says he's just playing but they don't like it. Doesn't think he has to respect his 24 year old sister. Gives her a hard time by being argumentative or not doing what he is asked when she's looking after him while I'm away. I told him he should care & protect his sisters not fight with them. I will say I can see that he's thinking about his actions after the fact which is a new improvement. He will apologize if asked. But I want him to think before he acts.
02. Behavior & academics at school
Slightly improved based on Schoology report since last report card.
03. Responds first time
No. More than twice.

Doing Well (5/5)
01. Behavior at home
Liam is 12 we have good days and not so good days. For the most part he is ok. Yes I have to say things for him to do twice and I yes I have to ttell him to do things he is not at the point of doing it when it needs to be done...i.e., trash cleaning his room
02. Behavior & academics at school
Well he was behind in his work so he had F's in 3 classes; As of today, he has completed the work he missed and his grades are no longer F's, as of last week he has a C in one class a B in the other and a D in the third class. I know he is capable of consistency when it comes to his school work. I recognize I must do better as well. Because when I got on his a$$ he got that work done and those grades up...so there is that
03. Responds first time

05. Emotional improvements
Liam does not show much emotion from jump but I do notice that he notices my emotions and is concerned about how I feel. When I ask about how he feels he is always..."okay" so I will continue to ask and listen; also he has a lot of life ahead of him and my hope is that Seed is being planted for such a time as emotions needing to be controled...if that makes sense.

02. Behavior & academics at school
In school he is doing okay. I had to get in his a$$ about hit laptop that he apparently dropped and has had a broken screen for I don't know how long. He did not tell me. For the most part good but the bad things he does is he teases his brothers so that I don't see what he's doing but they do in particular Malachi and it sets him off is causing him to have a ought against him. I have. Warned him that being neurodivergent.
His brother will Harbor that resentment literally cause he's not understanding, adjusting and teasing. It is literal for him and as a big brother, he should be setting an example of Grace.

01. Behavior at home
Judah, Malachi and Matthew are not throwing away their trash in their room. They still continue to eat upstairs when I’ve asked them not to and only to eat at the dining room table. There is even what appears to be pee in a pot bottle next to Judah‘s bed this is the first I’ve seen of this. I have to repeat take off your clothes put them in the dirty clothes and get into your night clothes when they get home Matthew being the longest to keep clothes on before bed. I’ve asked them not to have outdoor clothes in the house laying on they’re clean bed and sometimes piles of clothes that I have folded they will continue to lay on Malachi with over explaining when they are told to do something that continual needing to have the last say after they’ve been told to do something, Malachi continue complaining. I have explained to him that life and death is in what we say and to think about saying those things that are pure and lovely in a good report and not looking at the glass half empty he has amped up the complaining and the need to tell me how to do my job as a mother in fact, all three of them try to tell me how to discipline the others constantly telling me I know what to do I know, and I heard you already. I know I know all of my kids. I know it all they know already and they are very opinionated inappropriately expressing it when correction is taking place. They have that bad and slow to respond to instruction. Judah likes to argue and pick on his brothers causing strife Malachi feels like he’s being singled out and feels the need to be defensive and Matthew is screaming and yelling at me and telling me what to do
02. Behavior & academics at school
Academics are great for Matthew and Judah Malachi needs help. He needs constant explanation and instruction that he is not getting. I do not care for this teacher. I’m looking forward to seeing how a new teacher and a new environment. Going to sixth grade will do him. He is extremely intelligent, and I feel that if he had someone who understood his neurodivergent mind he would soar . He needs a lot of encouragement.
03. Responds first time
I have to repeat instructions to all three of them several times I’ve lost count They will even respond with. I’ll do it later. It’s not a no, but it’s a defiance and a passive no because they won’t do it when I say

01. Behavior at home
Behavior at home has declined significantly. There has been an increase in argumentative and defiant behavior, as well as poor decision-making. He has left the home without permission on multiple occasions, including going to a nearby location without notifying an adult. There has also been an incident involving unauthorized use of a sibling’s debit card to purchase items. Additional concerns include failure to complete assigned chores, dishonesty, use of inappropriate language in front of adults, and disruptive behavior during family routines (e.g., consuming a disproportionate amount of shared food without regard for others). These behaviors indicate decreased impulse control, poor judgment, and a lack of accountability.
02. Behavior & academics at school
He is currently returning from a suspension related to a physical altercation. There are ongoing concerns regarding peer interactions, including poor decision-making in friendships and noncompliant/disruptive behavior (particulary in unstructured settings- lunch/recess; hallway passing, etc). He has been described by staff as noncompliant at times. While there has been some improvement in academic performance, follow-through on assignments remains inconsistent. He also left school early recently citing a headache, which may be related to avoidance rather than a medical concern.
03. Responds first time
He does not respond to directions the first time. Instructions are frequently ignored or delayed, and tasks often remain incomplete. There is a consistent pattern of avoidance behaviors, including distraction, conflict initiation, or disengagement, which interferes with timely completion of responsibilities. Tasks assigned to him are left unfinished the majority of the time.

Struggling (0/5)
01. Behavior at home
Omari’s behavior at home has remained relatively consistent, with little to no notable changes in his social interactions with his brothers. He continues to engage with them in a typical manner (he purposely tries to annoy them or "ragebait" them)
However, over the past few weeks, his follow-through on household responsibilities has declined noticeably. He has required increased prompting and direct supervision to complete basic chores. In many cases, he needs to be physically redirected from his current activity to begin a task and remain on task. When left to work independently, tasks often remain incomplete, as seen in ongoing concerns such as an unmaintained bedroom and laundry not being managed.
Our goal is to support Omari in building greater independence and accountability, specifically working toward completing tasks and meeting expectations with minimal prompting, without delays or avoidance behaviors.

Our goal is to support Omari in building greater independence and accountability, specifically working toward completing tasks and meeting expectations with minimal prompting, without delays or avoidance behaviors.
02. Behavior & academics at school
Omari is currently serving a suspension related to a physical altercation at school. Leading up to the incident, school administration noted increasing behavioral concerns and difficulty maintaining appropriate boundaries. During the incident, he required staff intervention and later demonstrated challenges with following directions while awaiting pickup.
This is the first significant behavioral incident of this nature since late elementary school. While the situation is concerning and being addressed at home, it is important to note progress in his communication. Specifically, he did not use threatening or harmful language, which had been a concern in the past.
At this time, the full impact on his academic performance remains to be determined due to the suspension. Prior to the incident, he was maintaining mostly Bs and Cs, with a D in math.
Moving forward, our focus is on reinforcing appropriate decision-making, self-regulation, and accountability, while supporting him in maintaining consistent academic engagement.

03. Responds first time
Omari does not consistently respond to directions on the first request. In most situations, he requires multiple prompts and, at times, direct supervision to initiate and complete tasks. Independent follow-through is limited, and without continued prompting, tasks are often delayed or left unfinished.
There are occasional exceptions when the activity is highly motivating or directly benefits him (e.g., meals or preferred outings), in which case his response is more immediate.
Overall, he demonstrates difficulty with task initiation, sustained attention, and follow-through without adult support. Our current focus is on helping him build responsiveness to initial directions, reduce delays, and increase independence in completing expected tasks.

05. Emotional improvements
Prior to the recent incident at school and spring break, I believed Omari was beginning to show some improvement in emotional regulation, appearing calmer and somewhat more compliant at home. However, more recently, there has been an increase in combative and aggressive behaviors.
During our recent family trip, there were multiple physical incidents involving his younger brother, including one that resulted in a nosebleed and another that caused injury while the younger sibling was attempting to defend himself. In those moments, Omari demonstrated limited immediate remorse and difficulty de-escalating once upset.
That said, I did observe a notable positive shift later in the trip. While interacting with other children and being asked to help monitor his younger brother in the pool, he demonstrated increased care, attentiveness, and gentleness. This was a meaningful deviation from the behaviors seen earlier and suggests that, under certain conditions or environments, he is capable of more appropriate emotional responses.
Overall, his ability to regulate emotions appears inconsistent at this time. Our goal is to help him develop more consistent self-control, improve his responses during moments of frustration, and increase empathy and accountability following incidents.

Struggling (2/5)
01. Behavior at home
At home, there have been no significant changes in Omari’s behavior. He continues to experience difficulty with emotional regulation and impulse control.
02. Behavior & academics at school
At school, Omari continues to experience behavioral challenges, including receiving three lunch detentions since the last report. The most recent incident involved the use of inappropriate/profane language directed toward a substitute teacher after being accused of splashing water on peers; Omari acknowledged the language but denied the behavior. He also received a lunch detention for classroom disruption during another substitute-led class.
Academically, Omari’s grades show some improvement in certain subjects, with current grades of D in Math; C in Science and Social Studies; B in English; and A’s in Health and ASK. However, some grades have declined over the past two weeks due to late or missing assignments.

05. Emotional improvements
Omari has shown some improvement in his ability to recognize when he is becoming dysregulated and is making attempts to manage his emotions. However, he continues to struggle with consistently applying those strategies, and there are still moments where he is unable to maintain control. Additionally, there are times when he engages in behaviors intended to provoke others, which can lead to escalated emotional responses.

01. Behavior at home
His behavior at home continues to show improvement. He is increasingly able to self-correct when redirected. However, he still requires multiple prompts at times before completing tasks, and occasionally needs structured consequences (such as loss of privileges) to support follow-through. Overall, we are seeing gradual progress in responsiveness and accountability.
02. Behavior & academics at school
Academically, he is showing measurable improvement. This past week included several A and B grades, reflecting a positive upward trend from prior marks. Behaviorally, school reports indicate general stability with noted areas of progress, including successfully returning to eating in the lunchroom with peers. While baseline behavioral challenges remain, staff have acknowledged incremental improvement.
03. Responds first time
He does not consistently respond the first time he is asked. While he is showing improvement in self-correction, he often requires two to three prompts before initiating or completing tasks. On some occasions, additional structure or consequences are needed to ensure follow-through. We are continuing to work on increasing first-time compliance and independent task initiation
04. Sir/Ma'am
No
05. Emotional improvements
He is demonstrating increased self-awareness and improved emotional regulation. He is more frequently able to recognize signs of escalation before fully dysregulating and will sometimes self-correct without significant prompting. He is also becoming more aware of how his actions impact his brothers and parent, and he is quicker to adjust his behavior when redirected. Overall, he appears more receptive and agreeable during redirection compared to previous reporting periods.

Struggling (1/5)
01. Behavior at home
Behavior at home has been increasingly concerning and inconsistent. Omari struggles with responsiveness to directions, follow-through on responsibilities, and emotional regulation when frustrated. There are frequent power struggles around daily routines, and he often requires multiple prompts before complying.
He has, at times, made verbal threats of harm toward others during moments of anger. While these appear impulsive and emotionally driven rather than planned, they are concerning and not acceptable. He has also been sneaking access to his phone late at night despite boundaries, which is impacting sleep and likely affecting daytime behavior.
Additionally, there has been an increase in belligerent and disrespectful communication toward me, his grandmother, and his siblings. This represents a noticeable shift in tone and regulation at home.
I am concerned about the escalation in both intensity and frequency of behaviors and am seeking support to address the underlying emotional and behavioral factors contributing to them.

I am concerned about the escalation in both intensity and frequency of behaviors and am seeking support to address the underlying emotional and behavioral factors contributing to them.
02. Behavior & academics at school
Omari has received multiple behavior-related referrals at school, including the use of inappropriate language, disruption during lunch, and off-task/disruptive behavior in class (particularly Science). Administration has communicated that staff are feeling challenged in managing his behaviors consistently. There appears to be a pattern of impulsivity, peer-influenced behavior, and difficulty sustaining focus in less structured settings. I am concerned about both the escalation of behaviors and the potential underlying causes contributing to them.
03. Responds first time
Omari frequently requires multiple prompts before responding to directions, even when instructions are given clearly. Delayed compliance is consistent across both small tasks and more significant responsibilities. This pattern contributes to time management challenges, including difficulty leaving for school on time. I am working to establish clearer expectations and consistent follow-through at home, but I am seeking additional strategies to help improve responsiveness and accountability.
04. Sir/Ma'am
No
05. Emotional improvements
While Omari continues to struggle with emotional regulation and has frequent outbursts, I am beginning to see small but meaningful improvements. He is starting to recognize more quickly when his reactions are excessive and is ending his outbursts sooner than before. Although the frequency of episodes has not significantly decreased, the duration and intensity appear to be shortening. I see this as early progress, but continued support is needed to help him develop more consistent coping strategies.

Struggling (1/5)
01. Behavior at home
My son's behavior at home has been disappointing at times and pleasant at other times. He received a list of chores to do over the weekend that are pretty straightforward including clean the bathroom (that only he uses), sweep the floors, and about 8 other tasks. Out of the 10 tasks 7 of them were so poorly done that I made him redo them. It's very frustrating to see him do the things he wants to do (play video games) with great care and detail but when it's time to take care of his responsibilities he cuts corners and has to be monitored to get them done. The things he has done well with this week include being a gentleman and opening doors as we move around out in public, escorting me to the inner part of the sidewalk, he paid for his own haircut this week, and his behavior at school has been good. I very pleased to see these habits developing in him.
02. Behavior & academics at school
School is going well his teachers always have great things to say about him and his grades are showing improvement each card marking.

Doing Well (5/5)
01. Behavior at home
There has been a change in Thomas, and I’m unsure how to truly understand what’s happening inside him. He’s a wonderful child, but I’ve noticed a change in his behavior. He insists that everything is fine, but something feels amiss. As a single parent with limited family support, I’m doing my best to raise him, but I also recognize that doing it alone isn’t sustainable. I acknowledge that I don’t have all the answers and am open to seeking support and suggestions. My primary goal is to ensure Thomas’s success, but I also want him to be happy and healthy.
02. Behavior & academics at school
Thomas received his progress report since our last check-in, and unfortunately, he has an F in Language Arts and a D in Math. I was surprised because he had been improving. This is challenging because he has always been a great student until this 7th-grade year. I asked him if he needed tutoring or additional support. The teachers have given him some extra credit to complete, and he has time to improve before the final report cards are issued in April. I spoke with him, expressed my disappointment, and offered support. I would be interested in some tutoring resources if available. He mentioned that he is distracted, so I encourage him to advocate for himself and ask his teachers for help to avoid poor grades. I have also reached out to the teachers, and they say that he is capable of earning good grades, but he is not completing the assignments, both in class and homework. As a result, I have taken away his phone and video game privileges and set up more structured study time at home. I will also be more present for him. Previously, I didn’t need to assist him with his schoolwork because he was an honor student, but now I will be more involved.
03. Responds first time
He does not always respond when I first ask, and I have to repeat his daily chores to him all too often.

Doing Well (5/5)
01. Behavior at home
Great. He's always well mannered and kind, but I see maturity and growth.
02. Behavior & academics at school
His school behavior seems good overall, but he's struggling in his homeroom classes (Social Studies and Science). They are heavily project based. He previously had an IEP for Speech - Receptive and Occupational Therapy - handwriting, and is growing in the area of organization and focus (which are typically a challenge for him). Their are signs of neurodivergency in those areas. I'm going to contact the teacher again to see if his intervention teachers at the school can be more engaged. He's loves reading and learning, but is still developinging in clarity in expressing his thoughts, which is core requirement in writing papers and presenting.
03. Responds first time
He's typically congenial. Even when he doesn't want to do it, he complies. We do have to remind him at times, mainly because he gets easily distracted.
